To receive a Gaming Activity Report statement regarding your tracked play at Cactus Petes, please submit a request via email to cpwinloss@pnkmail.com or a completed signed Gaming Activity Report Request Form from the link below. Gaming activity information cannot be given over the phone, it must be in person at Casino Services, through email or by filling out the request form.

A completed Gaming Activity Report Request Form may be submitted for processing in one of two ways:
1. By mail to address listed on form.
*Notary will be required unless form is presented in person at Cactus Petes.
This includes the emailed requests!
2. In-person at the mychoice Center.
